The exposome
The word was coined in 2005 for the total of every environmental exposure across a lifetime, beginning before birth. Your genome is fixed at conception and can be read once. Your exposome changes by the hour — and almost nobody is reading it at all.

Air
Twenty thousand breaths, none of them chosen.

Water
Plastic, heat, and what leaches out.

Food
A supply chain you cannot verify.

Skin
An entry point, not a barrier.

Time
Working when the clock says sleep.

Threat
Vigilance with no recovery phase.

Six routes in, across the five recognised classes of occupational hazard — chemical, physical, ergonomic, temporal and psychosocial. Most of us meet all five before lunch.
Exposure by occupation
In 1700 Bernardino Ramazzini proposed adding one question to the medical history: what is your occupation? Find yours, and see what it exposes a person to — and which systems have to answer for it.
Illustrative only, and not a diagnosis — actual exposure depends on the workplace, the hours, the duration and the protective measures in place. Colours mark the five recognised classes of occupational hazard.

Who I see
Different starting points, the same method — find what the body has been responding to before deciding what to do about it.
Individuals
Employee, wife, mother, daughter, caregiver — often at once. Her body isn't handling one exposure. It's handling a stack. Perimenopause, metabolic and hormonal health, and the symptoms that arrive while reports still read normal.

Training hard on four hours of sleep. Clean lipid panel, nine-to-nine, and a body that isn't adapting. Capacity, recovery, and the difference between living long and living well.

When the exposure isn't chemical. Stress physiology, the HPA axis, and what chronic vigilance costs a body over years — measured properly rather than described.

A diagnosis you're not settled on, a report nobody explained, or a plan that hasn't worked. Brought back to first principles — history, evidence, and what the findings can and cannot support.

That sentence is where most of my consultations begin. “Normal” is a statement about a population, not about you — and it says nothing at all about what you've been exposed to.
Where you work, how you travel, what you breathe indoors, what you've eaten for a decade, what changed before the symptoms started. Longer than you're used to, because it has to be.
Testing chosen for your history, not a panel ordered by default. I'll tell you what each test can show and, just as importantly, what it can't.
Sequenced, specific, and built around a life you're not going to leave. Reviewed at intervals, adjusted with evidence.
The instrument
Ramazzini's question stayed in clinical practice for two centuries, then quietly disappeared. This is that question, rebuilt as a single page — free, and free for any physician to use. Message me and I'll send it across.
Training
Occupational medicine is where medicine learned to measure exposure. Functional medicine is where it learned to act on it. Very few physicians in India are trained in both.
